Would expanding Medicaid in Wisconsin really cost $1.1 billion?

This is important.  One thing missed here is that the state turns over the Medicaid/BadgerCare tax money to private insurance companies to "manage" the patients care.  The patient/applicant picks a plan. The state gives that insurance company $X per month to manage and pay for that person's care in that company's "network".  Seems to me that then the insurance company is setting the rates they pay to the care provider in their network.
