Under new health law, insurers target diabetics before they get lax in managing costly disease | Star Tribune:

Sounds like a good thing.  Community workers is a model that helps here.  MN has done some of this but the rates under Medicaid do not allow it to really work well.  Insurance companies, MCOs and the States need to expand this para-professional model idea for home visits, etc.  It works much better than a nurse calling me to bug me or getting a fancy colored mailing to bug me.  Too easy to ignore and it does not work like human contact from folks who "have been there".
